Learn how to create a structured form in MS Word, e.g. a document where users can only edit designated portions of the documents, useful for forms or creating standard project report forms.

details

Have you ever had the need to provide documents/template for your students where they responded to a structured set of questions? This demo show you how to create a "MS Word Form", where users can enter specific information in fields, select choices from drop down menus, or indicate choices by check boxes.

Some advantages of using this approach are:

* No special software beyond Word required (but students will need access to Word)
* The information returned is consistently formatted
* You can limit the amount of text returned
* You can create formatted numerical input (Date, time, decimal)
* With some elbow grease (and patience with the quirks of Word), you can create spread-sheet like calculated fields.
* The document can have portions locked from changes by the student
* The student can save the document to their hard drive and go back at any time to finish his/her work. (PDF forms read in Acrobat Reader do not save content)
* The student can save a copy of their document and/or email it to an instructor

We have been using this for the past year in the application forms for Faculty Professional Growth and the mcli Learning Grants. but there are adjust as likely some creative instructional uses you may dream up.

This was a great tutorial. It was very clear in its intent: making fillable word forms and the instructions were very simple. The only thing I would add to it to step the project's utlity up a notch is to provide a way for the teacher once she recieves these .doc to link each file to a scoring guide template. That way the teacher could send back the files to her students. Basically I realize that you are trying to use this "teacher quiz" senario as an example. I would like the example to go two steps further. 1. A method of linking each Space Report.doc to a scoring guide template. So the teacher could easily electronically grade each report and send the grade to each student.
2. Show how those grades could be linked to the teacher's grade book (an excell file).

That would be amazing. I think that would be a useful application of this fillable ms word tutorial with real world applications.
